This initiative came about after witnessing cases of reverts being buried according to non-Muslim traditions, and some even being cremated, because Muslims were unable to claim their bodies and fulfil their rights as fellow Muslims.
You can complete the document, print it yourself on certificate paper, and arrange for it to be signed with the Imam.
