Another example: If a Muslim is forced to break their fast, as we hear is happening to Uighur Muslims in China, they are allowed to break their fast in order to save themselves from being punished. And the correct opinion in this case (one who is compelled to break their fast) is that their fast remains unbroken. [Al-Majmū’ of An-Nawawi (6/325)]

If someone is excused from fasting and the their excuse is apparent, such as a child, the insane, someone who is obviously sick, etc., then they should not be reprimanded for eating infront of others. However, if the excuse is hidden, such as a menstruating woman, they must not eat in public in order to ward off any false accusations, or lest the ignorant ones are misled into thinking that it's permissible to break one's fast without any excuse.
